    We are seeking a compassionate and experienced nurse or homecare manager to join our team as a Care Manager. The Care Manager will work directly with our owners, playing a pivotal role in overseeing the delivery of care solutions and services to our clients, ensuring that their needs are met with the highest level of quality and professionalism. This individual will be responsible for coordinating care plans, assessing client needs, supervising caregivers, and serving as a liaison between clients, caregivers, and other healthcare professionals. We have a highly collaborative and supportive office team and are looking for a key team member who wants growth opportunities as our agency grows and who will fit well with our dynamic and fun office culture.

    Are willing to work occasional nights and weekends as client needs arise;
    Live in north county San Diego;
    Have a drivers license and reliable transportation.

    Responsibilities:


    Conduct comprehensive assessments of client needs and develop individualized care plans in collaboration with clients, their families, and other healthcare professionals.

    Coordinate and schedule care services based on client preferences, availability of caregivers, and specific care requirements.

    Recruit, train, supervise, and evaluate caregivers to ensure they deliver care in accordance with company policies, procedures, and standards.

    Conduct regular home visits to assess client satisfaction, monitor the effectiveness of care plans, and address any concerns or issues that may arise.

    Serve as a point of contact for clients and their families, providing support, guidance, and advocacy as needed.

    Collaborate with healthcare providers, including physicians, nurses, therapists, and social workers, to coordinate and integrate care services effectively.

    Maintain accurate and up-to-date documentation of client assessments, care plans, and service delivery in accordance with regulatory requirements and company standards.

    Participate in quality improvement initiatives and ongoing professional development activities to enhance skills and knowledge in the field of homecare.

    Be willing to serve as a back-up caregiver in the event of staff call-offs/uncovered shifts.

    Be willing to be on call and work occasional weekends and after-hour shifts as needed.

    Qualifications:


    Minimum of 5 years of experience in nursing, care management, as a caregiver, or a similar role in the healthcare industry.

    Bachelor's degree in nursing or related field highly preferred.

    Strong interpersonal, communication, and leadership skills with the ability to collaborate effectively with clients, caregivers, and administrative team members.

    Knowledge of California homecare regulations, policies, and procedures.

    Proficiency in computer skills, including Microsoft Office applications. Experience with care management and scheduling software highly preferred.

    Valid driver's license and reliable transportation.
