- Conduct Adult Protection screenings per agency, state and federal requirements for adults determined to be at risk due to allegations of abuse, neglect, or financial exploitation.
- Evaluates immediate (within 24 hours of receipt of report) and long-term safety through in person contact. Makes home visits to determine the needs of individuals for services and safety of adults, involving frequent travel to client locations, agencies, or provider locations.
- Ensures communication with core partners to streamline communication and effectively support the client. Identifies key services and resources, completes referrals and ensures the services commence. Coordinates with key partners like families, guardians, home-care providers, hospitals/staff, nursing home facilities, ombudsman's office, community resources, community mental health providers, schools, Performs highly critical social work duties in involuntary and voluntary adult protection cases.
- Implements and performs all assessment, investigation, and case management requirements, time reporting, documentation and monitoring strict timelines to meet state mandates and the needs of the client. This includes, but is not limited to, Structured Decision Making (SDM) tools, maltreatment determinations, assessment closing data, safety plans.
- Adjusting personal and professional schedules as needed to ensure safety.
- Determines eligibility for ongoing Vulnerable Adult case management. Assesses, develops and implements case planning and monitoring of services.
- Demonstrates understanding of state and federal statutes as they apply to each program.
- Completes continuing education to remain compliant with state and federal position requirements which includes keeping up to date on current research, methods, interventions, cultural inclusion strategies, and innovations in treatment modalities and funding sources. Training on being HIPPA compliant/confidentiality, mandated reporter practices, and evolving technologies.
- Review assessment results for Community First Services and Supports.
- Assist with developing McLeod County Agencies process for Community First Services and Supports.
- Create a Support Plan identifying the assessment results and chosen provider(s) for the Community First Services and Supports program and ensure a service arrangement is entered for the services per state and federal guidelines.
- Review and approve any changes in service needs throughout the year for the Community First Services and Supports program.
- Complete all related paperwork requirements as dictated by state and federal mandates and agency policies and in a timely manner.
- Contact Community First Services and Supports clients as required and as changes in the plan or state policy dictate. This includes verbal and written contact with the clients and service providers.
- Notify relevant agencies and inter-agency staff of significant changes concerning client cases.
- Complete time recording and case note documentation in SSIS according to agency standards.
- Maintain up to date information regarding DHS regulations, rules and procedures (i.e. researching, reading reviewing, and referring to manuals and bulletins).
- Attend training sessions, conferences, meetings, etc., on program specific issues, with prior approval of supervisor in accordance with personnel policies.
- Attend agency staff meetings as scheduled.
- Perform job related duties as assigned or apparent.
- Thorough knowledge of Adult Protection statutes, rules, policy manual and SDM tools.
- Knowledge of case management social work practices, assessment, plan development implementation, monitoring, and evaluation procedures for VA/Adult Protection services.
- Ability to listen, comprehend, and effectively communicate information both written and orally to all individuals.
- Ability to write and complete case documentation and reports.
- Ability to communicate with, work with, and understand the problems faced by families from all cultural and economic levels.
- Ability to work with and motivate the most disadvantaged segment of the community.
- Knowledge of individual and group behaviors.
- Knowledge of child welfare and vulnerable adult laws relating to abuse and neglect.
- Knowledge of related court procedures including due process of law and rules of evidence.
- Knowledge and ability to use appropriate resources and services.
- Knowledge of human development and the impact of maltreatment.
- Ability to apply knowledge and coordination with specific Community First Service and Supports providers.
- Ability to deal with customers who are under stress, and who may be angry, hostile, verbally aggressive
- Ability to both immediately respond to high safety needs as well as plan and monitor long-term safety planning.
- Ability to effectively apply engagement strategies including solution focused interviewing and/or motivational interviewing skills.
- Bachelor of Arts/Bachelor of Science in Social Work or Human Services-related field plus one year of professional work experience required or;
- An equivalent combination of education and experience sufficient to perform the essential functions of the job.
- Valid Minnesota driver's license with proof of insurance.
- Families needing protective services or requesting adult welfare services to determine the safety of vulnerable adults, risks of abuse or neglect, and strengths and needs.
- County Attorney, court personnel, and Case Managers, Public Health and client advocate with other workers' case plans.
- Minnesota Adult Abuse Reporting Center.
- Various agencies, businesses and providers throughout the state, including hospitals, regional treatment centers, health plans etc.

- Work is mainly performed in an office setting but may also be in clients' homes, residential facilities, and other county offices.
- Frequent travel may involve exposure to inclement weather conditions, unsanitary conditions in homes and unsafe environments.
- May require working with people who may be disoriented, hostile, aggressive, abusive, or threatening.
- There may be hours worked outside of the normal business hours.
- Desk work involves use of a computer, telephone, and other devices.
- Communicate effectively with County staff, clients, and professional groups.

Description
Supplemental:This is a split position that will work with individuals referred for Adult Protection Services (APS) and Community First Supports and Services (CFSS).
APS: to work as part of our APS team to investigate reports of vulnerable adult maltreatment, provide protective services to resolve and determine allegations of abuse, neglect, and financial exploitation within McLeod County area.
CFSS: to work with a caseload of individuals who have been determined eligible to receive this service, complete support plans as required and ensure the needed service agreements are entered for the service to be paid.
