Program Overview
Oak Street delivers high-quality primary care to Medicare patients in their home. The @HOME program takes a team-based approach to providing outstanding patient care.
Our Care Teams - consisting of a Provider, Medical Assistant/Scribe (CIS/MA), Behavioral Health Specialist (BHS) and Mobile Operations Coordinator (MOC) - build individual relationships across a panel of about 75 patients, seen monthly.
The Care Team is responsible for delivering excellent, high-touch primary care, and coordinating the care of our patients throughout the healthcare delivery system.
Role DescriptionThe Data Analyst will create ad hoc analyses and reports that interpret and analyze data trends for Oak Street Health At-Home.
This role involves producing data reports to support programmatic decisions and maintaining periodic reports that aid in assessing program performance.
Additionally, the Data Analyst will contribute to identifying areas for future analysis and metrics, as well as developing impactful performance indicators, thereby playing a key role in enhancing program effectiveness and strategic planning.
Core ResponsibilitiesIdentify, analyze, and interpret trends or patterns in complex data sets.
Handles all data requests and special analyses for internal and external partners.
Partner with leadership and care teams to identify improvement opportunities within healthcare programs. Utilize data-driven insights to propose strategic initiatives, optimize program performance, and achieve superior patient outcomes.
Actively participate in strategic planning sessions, offering analytical support and recommendations to guide the direction and priorities of the program.
Assists in the development and monitoring of key performance indicators to evaluate the effectiveness of operational changes and the overall performance of the program.

Required Qualifications2-4 years of relevant work experience in Data Science, Business or Healthcare Analytics.
Undergraduate degree, preferably in Computer Science, Business or Analytics
Advanced Excel Proficiency:
Applicants must demonstrate advanced Excel skills, particularly in utilizing Power Query, mastering advanced functions, and leveraging PowerPivot.
Proficient in
SQL:
The candidate must possess basic SQL skills, including the ability to write simple queries, understand database structure, and manipulate data.
The ability to read and write Python code is preferred.Candidates with skills in Python programming will have an advantage, as this capability is valuable for automating tasks, performing data analysis, and developing algorithms.
An analytical mind and inclination for problem-solvingBusiness process understanding, preferably within the healthcare sector.
Demonstrated ability to collaborate effectively in a team setting
