    Job Title: Weekend Registered Nurse (RN) - Home Health Visits

    We are seeking an experienced and compassionate Registered Nurse (RN) for a per diem/PRN nursing position in Raleigh, North Carolina.

    About the Role:

    This is an ongoing per diem/PRN weekend position with our Raleigh/Durham Senior Living office. The successful candidate will work with adult and geriatric clients on a per visit basis within senior living communities throughout Wake and Durham counties.

    The ideal candidate will have home health experience and be available to work a weekend on-call schedule from Friday evening at 5:30pm through Monday morning at 8:30am, for phone triage, admissions, and visits as needed.

    Responsibilities:

    • Follow a designated care plan in accordance with patient's needs
    • Make home visits to clients in designated geographic territories
    • Perform assigned duties, including administration of medication, wound care, treatments, and procedures
    • Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
    • Follow up with, execute, and properly document doctors' orders
    • Perform client assessments as necessary
    • Case management and coordination
    • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet

    Requirements:

    • A current license as a Registered Nurse in North Carolina
    • A minimum of one year of recent, verifiable clinical (medical/surgical) experience
    • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
