- Adept at listening and clarifying issues for effective problem-solving
- Manage schedules for all assigned patients, ensuring timely adjustments when necessary
- Handle calls and address queries related to patient and caregiver schedules, escalating clinical matters when required
- Coordinate various types of visits such as Admission, Recertification, Resumption, and Discharge
- Follow up on missed visits with clinicians to reschedule as needed
- Ensure compliance with visit frequency orders for patients
- Assist with staff and patient statistics and reporting
- Participate in mandatory HIPAA/Privacy Program and Employee Compliance Program
- Bachelor's degree or equivalent work experience
- Proficiency in Windows environment, particularly Outlook, Excel, and Word
- Background in Social Work, Human Services, Psychology, or Business
- At least 1 year of experience in a home health setting
