    Job Description

    This role provides behavioral health support to residents of long-term care facilities under the supervision of a licensed professional.

    Key Responsibilities:

    • Conduct comprehensive assessments of patients' behavioral and social needs.
    • Coordinate care with families and facility staff as recommended by licensed professionals.
    • Educate patients on their clinical condition and provide self-management support.
    • Ensure treatment adherence through outreach to clients experiencing challenges.
    • Assist in developing individualized goals and behavioral health treatment plans.
    • Collaborate with other agencies and community providers to ensure seamless care.
    • Maintain accurate and timely documentation of work.
    • Act as an active member of the behavioral health treatment team.

    Requirements:

    • Bachelor's degree in Counseling, Social Work, Psychology, or related field.
    • Three years of post-degree experience in a psychiatric or long-term care setting (preferred).
    • Able to function independently and proactively in a long-term care setting.
    • Effective use of electronic medical records.
    • Strong written and verbal communication skills.
    • High degree of initiative, judgment, and discretion.
    • Ability to handle confidential and sensitive information.

    Work Environment:

    The work environment is collaborative, and employees must be able to interact effectively with others in an office setting. The noise level is usually low to moderate. Employees are expected to possess a respectful and professional demeanor at all times.
