    QIDP

    [Qualified Intellectual Disabilities Professional]

    BASIC FUNCTION

    Responsible for the day-to-day delivery of individual support program services to assigned case load in accordance with Federal, State, and facility codes, regulations, guidelines and expectations.

    RESPONSIBILITIES

    1. Develop and implement Individual Support Plans [ISPs] for each person in assigned case load based on accurate, reliable assessments and other information.

    2. Ensures the ISP addresses specific needs, etc., as identified from CFAs [Comprehensive Functional Assessments].

    3. Ensures programs are written to meet visions and goals identified on the resident's ISP.

    4. Ensures written programs meet regulatory and facility requirements prior to implementation.

    5. Recommends and develops programs for unaddressed priority needs.

    6. Ensures that data collection forms and collection methodology reflects specific progress to stated program objective completion criteria and/or other outcomes for which the data is being collected, i.e., sleep data, participation data.

    7. Directs and ensures the delivery of ISP services.

    8. Systematically analyzes data to determine status related to criteria in the objective.

    9. Recommends and implements timely modifications of unsuccessful training programs

    10. Actively participate in facility Interdisciplinary Teams [IDT].

    11. Design and implement staff development programs [in-services] related to program services for all staff in all departments to maintain and improve their competencies.

    12. Ensure residents' personal hygiene, dress and grooming is acceptable by local expectations.

    13. Spends most of the workday in the program areas training and directing, supporting the trainers.

    14. Ensures continuous active treatment regimen for those individuals assigned in the caseload.

    15. Any other responsibilities assigned by supervisor and/or administrator.

    DUTIES

    1. Develop, recommend, and implement support needs, for each individual based on accurate, reliable assessments that reflect the individual's needs.

    2. Monitors programs for success and recommends and implements timely modification of unsuccessful programs.

    3. Design and implement staff development programs and plans to increase the skills and competencies of all personnel in the provision of services.

    4. Conduct and actively participate in the facility inter-disciplinary team.

    5. Frequently visits program and supervise the active treatment program areas (both on and off facility grounds) discussing performance and problems of individuals. Develop or recommend actions to correct or improve these areas. Document actions, activities and results.

    6. Systematically record and analyze collected data to justify program inceptions, changes, or terminations in a timely an accurate manner.

    7. Recommends and develops support programs for unaddressed but significant needs.

    8. Make recommendations and requests on behalf of the individuals in the caseload.

    9. Work directly and effectively with others who work with the individuals.

    10. Maintains a liaison with the individuals, their families, support personnel, etc., to assure that the individuals needs are continually addressed and met.

    11. Ensures ISP are developed per policy and procedure and implemented within seven (7) days of the IDT meeting.

    12. Reviews all data monthly for accuracy and currency as evidenced by the QIDP initials and date of review on each data sheet in the QIDP's caseload.

    13. Ensures, by observation and interviews that all staff implementing support programs in your assigned caseload are trained by the Responsible Person [RP] named on the programs to teach the programs and document data.

    14. Ensure that each individual in your caseload has functional and age appropriate materials available throughout the day at all locations/areas.

    15. Ensure continuous active treatment for individual in your caseload is evident by staff working with individuals demonstrating competency in the following techniques.

    A. Using "teaching opportunities" at all times to ensure individuals are continuously being exposed to new skills and information;

    B. Redirecting self-stimulatory behavior;

    C. Providing continual and positive interactions with individuals; Utilizing group teaching methods to keep all individuals involved in a functional activity.

    16. Ensure individual twenty-four hour schedules are developed and staff are trained and can competently implement schedules.

    17. Ensure individual schedules identify "CARRYOVER" programs and that these programs are implemented.

    18. Utilize the corporate staff and facility consultants as resources for technical assistance in implementing any of your job duties and/or expectations.

    19. Be supportive of corporate and the facility management in changes that occur as evidenced by your cooperation and teamwork with other staff.

    20. Assist in coordinating and monitoring the facility's Quality Assurance and Performance Improvement [QAPI] program.

    21. Communicate all observations to the Program Service Director [PSD] and QAPI committee.

    22. Any other duties assigned by your supervisor and/or administrator.

    MARGINAL FUNCTIONS

    1. Performs clerical functions, such as completing forms, reports, etc., as required.

    2. Assist with orientation of new employees in relation to program services.

    3. Participates in professional organizations, associations, activities and programs related to the position.

    4. Creates and maintains an atmosphere of warmth, calm, personal interest and a positive attitude and treats everyone one with dignity and respect.

    5. Maintains a professional image in appearance and in the office/work environment.

    EXPOSURE RISK:

    Is at medium risk of exposure to blood and body fluids when actively performing job duties and functions.

    SUPERVISION RECEIVED:


    Supervised by the PSD [Program Services Director]


    SUPERVISION EXERCISED:

    Provides programmatic supervision to Direct Support Professionals [DSPs]

    QUALIFICATIONS

    · EDUCATION

    Is one of the following: A doctor of medicine osteopathy, a registered nurse, an individual who holds at least a bachelor's degree in a professional category specified in the Federal regulation paragraph (b)(5) or designated as a human services professional that has at least a bachelor's degree in a human services field (including, but not limited to: sociology, special education, rehabilitation counseling, and psychology).

    · EXPERIENCE

    At least one year's experience working with intellectual disabilities or other developmental disabilities.

    · GENERAL

    When State licensure and/or certification is required for the professional degree held by the QIDP, the license must be current and unencumbered

    WORKING CONDITIONS

    1. Sits, stands, bends, lifts, and moves intermittently throughout the workday.

    2. Subject to frequent interruptions.

    3. Attends ongoing educational experiences pertaining to the position and/or required for licensure/certification of the professional degree to keep current with trends in the field.

    4. May be subject to infectious diseases, waste, conditions, etc., including the potential exposure to HIV and HEPATITIS B viruses.

    5. Works beyond normal 8-5 working hours, weekends, holidays, other positions and various shifts.

    6. Subject to falls, cuts, bruises, burns from equipment, odors, etc., throughout the working day.

    7. Is subject to hostile and emotionally upset clients, family members, personnel, visitors, etc.

    8. Generally will work in an air-conditioned, heated and ventilated building and may work outdoors

    SPECIFIC REQUIREMENTS

    1. Must be able to read, write and speak the English language in an understandable manner.

    2. Role model professional behavior by performing and/or assisting other staff, supervisors, peers, consultants and direct support staff in an effective, cooperative manner, in all duties.

    3. Must be of good moral character.

    4. Must possess the ability to deal tactfully with all consumers, personnel, residents, families, and visitors, etc.

    5. Must be able to follow written and oral instructions.

    6. Must have the ability to work harmoniously with other personnel and be willing to manage persons of various maturity and cognitive abilities.

    7. Must have personal integrity, have flexibility, and the ability to work effectively with others.

    8. Must have and maintain a valid State of Mississippi drivers' license and meet the criteria to be insured with the company's motor vehicle insurance. (This requirement is waived for all current employed QIDP that does not meet this requirement and was hired prior to 11/11/2013).

    PHYSICAL REQUIREMENTS (With or without the aid of mechanical devices)

    1. Must be able to sit, stand, bend, and lift without assistance at least 25 pounds.

    2. Must be able to move intermittently throughout the workday.

    3. Must be able to read, write and speak the English language in an understandable manner.

    4. Must be able to cope with the mental and emotional stress of the position.

    5. Must possess sight/hearing senses, or use prosthetics that will enable those senses to function adequately so that the requirements of the position can be fully met.

    6. Must be in good general health and demonstrate emotional stability.

    7. May be required to lift, push, and pull.

    8. Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times, hostile persons including residents, family members, visitors, etc.

    9. Must function independently without direct supervision.

    10. Must maintain good grooming, personal hygiene skills and appearance
