- Responsible for accurate and detailed data entry into the electronic health record.
- Scan and file medical records, correspondence, and other documentation into patients charts in electronic health record.
- ROI - Release of medical records to requestors including patients and other medical facilities in accordance with HIPAA, State and Federal rules and regulation. Collect fees when applicable.
- Protect patients rights by always maintaining confidentiality of personal and financial information and in accordance with HIPPA regulations.
- Escalate any patient related issues and delays in timely release of records to manager.
- Assist patients with completion of release of information form.
- Scan/upload patient information to electronic health record.
- Answer the telephone.
- Assist in other duties as assigned.
- High School Diploma or equivalent
- One year of customer support experience in Medical Records required
- Two years of healthcare industry experience preferred
- Microsoft Office experience preferred
- Excellent verbal and written communication skills

As the Health Information Management Specialist for Swope Health Services, you will be apart of a team that supports an organization that provides care to the people who need it the most. The Health Information Specialist performs a variety of tasks associated with the collecting, analyzing and indexing of patient care documentation in the medical record applications. Other responsibilities include the process to store, retrieve, review, analyze, and process health-related information. The Health Information Specialist is responsible for all functions of the Health Information Processing Specialist along with additional complex operations including indexing of all documents received electronically, obtaining documentation from third party portals.Primary Functions:
