    Responsibilities:

    Position Summary:

    To assist in the coordination of and support the development of the diabetes management/educational services, ensuring quality patient care that complies with the goals and objectives of Sequoia Hospital and Dignity Health. To expand the community service capability of the Northern California Diabetes Institute (NCDI) at Sequoia Hospital by providing outpatient and inpatient screening, assessment, and counseling of diabetes self-management skills. In accordance with the mission and philosophy of Dignity Health, the diabetes educator at Sequoia Hospital is a professional caregiver who assumes responsibility and accountability for the assessment, planning, implementation and evaluation of care for patients and their families.

    This position is per diem to cover for vacations and potentially one day a week of additional coverage for the Diabetes Education and Nutrition Center.

    Qualifications:

    Minimum:

    • Minimum of one year experience in direct diabetes care or care of the chronically ill patient.
    • Bachelor's Degree in dietetics
    • California Registered Dietetic (RD) license
    • Valid California Driver's license in good standing

    Preferred:

    • Bilingual preferred.
    • Certified Diabetes Educator credential designation preferred.
    • Experience with Diabetes & Pregnancy management preferred.
