CLIENT'S SUMMARY OF RESPONSIBILITIES*Collaborate with client in the formulation of an Individual Recovery Plan that is reviewed and modified with clients on a regular basis*Provide outreach, case management, counseling, advocacy, and other needed services to clients in any environment including:
the streets, shelters, jails, hospitals, apartments, office etc.

80 percent of contact with the clients is in the field*Maintain written and computerized records, compile reports and complete other program documentation( ex:
progress notes, incident reports, on-call logs, letters and psychosocial assessments)*Lead groups, workshops, and in-service meetings on topics relevant to identified areas of client need*Perform related work as assignedQualificationsDC Licensure as a Certified Addiction SpecialistMinimum of one year experience in social services, supported housing, or with persons with disabilitiesDriver's License and vehicle requiredExcellent communication, writing and computer skillsCovid 19 VaccinationCOMPANY OVERVIEW Delta-T Group's mission is to provide cost-effective, reliable, and innovative staffing solutions for social services, behavioral health, allied health and special education staffing, for the betterment of thoseneeding care and support
