About the Role

As a Licensed Practical Nurse (LPN) at Providence, you will be an integral part of our interdisciplinary team providing whole-person quality care to patients in their homes.

Key Responsibilities

Provide direct patient care and develop individualized care plans to meet the unique needs of each patient.
Work collaboratively with other healthcare professionals to ensure seamless delivery of care.
Maintain accurate and detailed records of patient interactions and progress.

Required Qualifications

Certification as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of California.
National Provider BLS - American Heart Association certification within 30 days of hire.
1 year of experience in nursing, hospice, or a related health field.
