    Description

    Job Duties:

    • Provides services to persons with severe and persistent mental illness who qualify for services under the Intensive Community Outreach and Recovery Team (I-CORT) program and serves as a member of the I-CORT multidisciplinary team.
    • Assesses, plans, develops, coordinates and provides treatment for individuals enrolled in the I-CORT program.
    • Provides therapy and case management activities for individual enrolled in the I-CORT program, including coordinating and monitoring the activities of the multidisciplinary treatment team. Assigns primary point of contact/service coordinator for each person enrolled in the I-CORT program.
    • Conducts comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and co-occurring issues.
    • Ensures multi-disciplinary team provides peer support services, therapy, medication administration/monitoring, general health care monitoring/treatment, supportive counseling, social/hygiene skills training, recovery/resiliency support, symptom management, budgeting skills, and leisure time activities.
    • Provides support to individuals, both on a planned and as-needed basis, to help them accomplish their personal goals, to cope with the stressors of day-to-day living, and to reach goals as identified on the ISP and Recovery Support Plan.
    • Provides services in the individual's natural/community setting 75%-85% of the time.
    • Collaborates with Crisis Stabilization Unit (CSU) and Intensive Community Support Specialists (ICSS) to identify individuals that may be appropriate for services under the I-CORT program.
    • Facilitates daily organizational meetings to review service contacts within the past 24 hours, develop action plan for the current day, and revise plans of care as needed.
    • Participates in agency MAP teams and staffings when indicated.
    • Maintains client confidentiality at all times.

    Documentation and Quality Improvement:

    • Documents appropriate activities in case note format within specified timelines.
    • Maintains case record documentation that is adequate, appropriate, complete, and timely.
    • Completes and submit service logs/documentation per agency policy.
    • Develops the Individual Service Plan and Crisis Support Plan with input from the multi-disciplinary team.

    Interpersonal and Leadership Skills:

    • In conjunction with the County Supervisor, participates in the hiring, training, and evaluation of competent program staff.
    • Maintains an on-call schedule between team members to ensure availability to respond to needs of I-CORT participants after hours.
    • Provides clinical direction, supervision, and training for all program staff.
    • Reviews and approves time sheets for staff under direct supervision.
    • Maintains monthly evaluations of probationary staff under direct supervision. Participates in the annual evaluation process of staff under direct supervision.
    • Identifies and collaborates with the HR department for the clinical training needs of staff.
    • Monitors, tracks, and ensures identified goals as outlined in the grant are met. Submits reports as required and in accordance with established timeframes.
    • Ensures that team members provides services to each individual as often as necessary, but no less than two times/week.
    • Responds to consumers' needs in an appropriate and timely manner.
    • Demonstrates appropriate interactions with coworkers that promote a pleasant work environment.

    Support Duties:

    • Seeks professional certification and/or licensure and clinical training opportunities.
    • Completes additional job duties as assigned by supervisor.
    • Participates in interagency collaboration for the provision of services.

    Key Responsibilities
    ESSENTIAL QUALIFICATIONS/SKILLS/


    EXPERIENCE:
    Clinical licensure or DMH Certified Mental Health Therapist within 30 days of hire and maintained throughout duration of employment
    Available 24/7 as assigned to respond to needs of individuals enrolled in the I-CORT program.
    Requires excellent time management, organizational, and interpersonal skills.
    Requires satisfactory completion of CPR prior to reporting to the work site.
    Requires satisfactory completion of CPI within 30 days of employment.
    Requires valid driver's license and current auto liability insurance.
    Requires ability to transport clients in private vehicle when necessary.
    Requires completion of annual training in compliance with DMH standards.
    Requires working knowledge of the DSM.

    PREFERRED QUALIFICATIONS/SKILLS/


    EXPERIENCE:
    Previous experience as a therapist in a community based setting strongly preferred.
    Previous supervisory experience preferred.
    Previous experience working with co-occurring disorders.

    Minimum Requirements
    Master's degree in a behavioral science. Clinical licensure or ability to obtain.
