- Collaborate with providers, nurse, social services, ensuring health care maintenance is current on empaneled patients, conducts assessment, screening and interviews, patients clinical, psychosocial and functional status.
- Coordinates visits with assigned providers, conducting pre visit planning, proactively identifies and coordinates pt. care, interacts with the patient/family/caregiver and provider to discuss pt. care, participates in clinical initiatives to improve pt. health.
- Participate in department orientation, quality improvement programs, provides direct nursing care.
- Processing notifications and writing referrals as needed, documents efficiently and in timely manner, ensuring that there are no deficiencies.
