    Overview

    The Program Social Worker (SW) is assigned to therapeutic preschool, pediatric medical program or adult medical program.

    Job Responsibilities:

    • Perform comprehensive and timely psychosocial assessment with program participant/legal guardian at admission.
    • Orient program participant/legal guardian to the program services, policies and process at admission.
    • Attend program care team meetings to track progress; notify team of barriers to successful outcomes related to participants psychosocial background, obtain team report on progress, expected discharge date, resource needs and concerns related to participants outcomes.
    • Provides participant/legal guardian with timely updates from care team conferences including update on expected discharge date from medical programs, problem solve barriers to outcome goals, identify and provide resources, and facilitate referral to community resources.
    • Set family conferences at the recommendation of the care team including participant/legal guardian and care team, serve as discussion facilitator in soliciting team progress report and ensuring exchange of information between team and participant/legal guardian and provide participant/legal guardian with written summary.
    • Provide emotional support and education to participants/legal guardians.
    • Assess the need for individual/family counseling services or neuro-psychological assessment and facilitate referral to neuro-psychological and rehabilitation psychology services in the agency or community as appropriate.
    • Provide in-services to rehabilitation staff on psychosocial issues relevant to the population served.
    • Serve as contact and facilitator for staff in mandated reporter events.
    • Complete additional duties assigned by supervisor
    • Participate in implementing regulatory guidelines for assigned programs as determined by supervisor.

    Qualifications:

    • Bachelor's or Master's in Social Work
    • Pediatric experience
