Job summary
The evening case manager performs duties relating to case management including assisting clients in obtaining supportive services within community to meet client needs. facilitates individual and group services.- Assesses needs of clients to link to community resources to meet client needs.
- Assists in obtaining supportive services to aid the client in their personal goals in recovery through the services of the agency or through referral to other agencies or programs.
- Furnishes education on daily living skills within the supportive housing environment.
