Job Description:
The Licensed Practical Nurse (LPN) provides skilled nursing care to the ill or injured in home, hospital, or extended care facilities. Working under the direction of the Nurse Case Manager and/or Clinical Manager for home care cases and shift supervisor for institutions.

Essential Responsibilities:
- Coordination of care with community and agency resources
- Receive and report on client assignments and perform procedures accurately
- Communicate care issues promptly to the designated registered nurse or CM
- Assist clients in meeting socialization needs and promote education
- Ensure timely and accurate documentation of patient/job information
- Communicate effectively with clients of all ages and backgrounds
- Maintain punctuality, appropriate time off usage, and fulfill annual education and medical requirements
- Perform other duties as assigned by Clinical Manager/designee

Minimum Requirements:
- Graduate of a NLN accredited school of nursing
- Current State of Connecticut Practical Nurse license
- One year of acute care experience
- Previous homecare experience preferred
- Maintain a current Drivers' License and reliable transportation
