    Patient Care CoordinatorFull-timeESSENTIAL DUTIESFunction as a member of the Care Coordination Team by providing care management and coordination for patientsAssist in creation of new workflows for Care Coordination TeamCoordinate and assist in patient schedulingCoordinate follow-up with medical teams, social services and community resources if neededUnder direction of medical staff, facilitate specialty referrals, including making necessary appointments and patient contact;Contact referral resources to ensure necessary reports are received at the centerHandle patient correspondence, as necessary.

    Coordinate with Team to reach goals for designated metrics and close specified gaps in careWork closely with Registered Nurse Care CoordinatorsMaintain patient records and provide record transfers, as necessary and in compliance with HIPAA regulations, policies, and proceduresMaintain accurate statistical information, including documentation to track patient compliance with referrals and prepare reports, as necessaryAs directed, provide information and data for reportsParticipate in outreach to new and existing patients when necessaryIncludes pediatric immunization and CHDP exam remindersOrder immunizations for all applicable programs.

    Check accuracy of vaccines being logged and send County quarterly reports.
    When needed, make patient appointments for specialty careCoordinate with the County representative on CHDP issues, updates, and changes.
    Assist in County and PPMM audits.
    Order and maintain CHDP pamphlets and handouts for pediatric visits.

    Perform pre-visit planningParticipate in daily huddles and in team meetingsContribute to achieving health center productivity goalsUse clear and culturally sensitive language when communicating with patients, staff, and outside providersUse medical interpreters to communicate with patients, if not proficient in patient's preferred language.

    A minimum of one year Health Services Specialist/Medical Assistant experience;Care coordination experienceCase management/care coordination certificate or coursework can substitute for experienceBachelor's Degree, or;Combination of experience and college education equaling 3 years, or;Graduation from high school in conjunction with a medical assisting certificate.
