    Description
    Southbrook Healthcare Center, a member of the CommuniCare Family of Companies is currently seeking a Transportation Assistant to join the team. This role involves assisting residents with transportation to and from both inside the facility and outside physician appointments in a safe manner. The position is integral in ensuring quality care delivery, supporting team members, and celebrating team successes. **JOB DUTIES & RESPONSIBILITIES** - Transport residents to outside physician visits safely. - Assist with transportation to scheduled specialty appointments within the facility. - Transport residents to and from in-house activities. - Provide one-on-one monitoring for residents at risk during outside visits. - Perform basic nursing care procedures as needed. - Maintain a clean, comfortable, and safe environment for residents. - Keep resident documentation records updated. - Adhere to established policies, procedures, and quality standards. - Participate in training and educational programs. - **QUALIFICATIONS & EXPERIENCE REQUIREMENTS** - High school graduate or G.E.D. equivalent. - Valid Driver's License required. - Completion of an approved Nurse Aide Training Program or eligibility to enter such a program. - Possession of Red Cross First Aid Certificate. - Previous experience in independent decision-making roles. - Previous healthcare or service industry experience preferred. - Compassionate and enthusiastic personality. **Benefits** - Competitive wages and PTO plans. - Benefit options including life, disability, medical, dental, and vision coverage. - 401(k) with employer match and Flexible Spending Accounts. **About Us** - CommuniCare is a family-owned company offering post-acute care services nationwide.
