As a Home Health Physical Therapist, you will:
Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
Assess and document patients physical strengths to support physician evaluation and guide treatment goals.Develop and implement a rehabilitation program consistent with physicians Plan of Treatment and the goals of the patient team.
Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and
patient observations, interventions, and evaluations promptly and thoroughly.
Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
Use your skills to make an impactRequired Qualifications:
Degree from an accredited Physical Therapy Program (approved by the APTA)
Current and unrestricted Physical Therapy licenseCurrent CPR certificationExperience collaborating with a team of healthcare professionalsPreferred Qualifications:
One year of experience as a Physical TherapistPay Range
Driving Statement:
This role is part of Humanas driver safety program and therefore requires an individual to have a valid state drivers license and are expected to maintain personal vehicle liability insurance.
Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher.
Scheduled Weekly Hours1Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting.
