The PACE Caseworker plays a vital role in supporting participants and families by coordinating services in alignment with the PACE model of care. The position involves developing and implementing participant care and discharge plans, navigating new enrollees through the program, providing psychosocial casework and referrals, and collaborating closely with an interdisciplinary team to provide outreach education and counseling services.

The ideal candidate will have experience working in multicultural settings required. At least two years of experience with Care Coordination Discharge Planning Utilization Review or similar is preferred as well as bicultural or bilingual skills depending on the population served.
