    Position Summary:

    They will be responsible for member outreach and retention, overall patient experience and acts as a resource for members . Is available to help patients understand the medical group practice as well as select the appropriate primary care provider. Assists in capturing correct patient demographics and insurance information for the practice. Functions as a resource to facilitate patient care needs in the clinic and acts as part of the patient experience team.

    Essential Duties and Responsibilities include the following:
    • Assists in contacting all new and current members to facilitate their care
      Educates Medical Group members on their health care team
    • Assists with any concern or issues regarding customer service; including feedback regarding the office and/or PCP visit, changing PCP, referral process, resources etc.
    • Provides service recovery by facilitating member's needs by working directly with appropriate Office Manager and staff
    • Meets one on one with members to educate and support them whenever possible
    • May assist patients with various appointments and scheduling
    • Conducts patient experience initiatives as needed
    • Maintains routine communications with members providing timely follow up to ensure issues are resolved
    • Educates members on clinic processes and programs
    • Other duties as assigned by management
    The pay range for this position at commencement of employment is expected to be between $ $27.00 per hour; pending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.

    The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

    Requirements

    Education and/or Experience:
    • Successful completion of High School with Diploma, Bachelor's Degree Preferred
    • Minimum 3-4 years' experience in health care or social services sectors
    • Excellent verbal, written, and interpersonal communication skills
    • Proficient at Microsoft Office products
    • Ability to work independently
    • Bi-lingual Spanish /Armenian required
    • Must successfully pass language skills test
    • Travel Required with the ability to travel throughout Los Angeles, San Fernando Valley, Santa Clarita, Simi Valley and Ventura County
