Under the direction of the HIV Program Manager, the Housing Opportunities for People with AIDS (HOPWA) Case Manager is accountable for providing intensive case management and crisis intervention services for those affected by HIV/AIDS.+

Responsibilities

  • Maintain confidentiality as per the Confidentiality Agreement.
  • Conduct initial assessments to determine client eligibility and needs (via phone and in-person).
  • Create and maintain service plans quarterly with well-developed goals using motivational interviewing and trauma-informed practices.
