- Develops an understanding of the child or youth's needs for environment and caregiver, wants in a placement, personal strengths and coping skills, cultural beliefs and creates and individualized referral to make the most appropriate match for the child or youth transitioning from a higher level of care (PRTF, QRTP, YRC)
- Updates the individualized referral timely and it is comprehensive as well as representative of the child currently
- Participates in all discharge meetings; when unavailable locates alternate person to participate
- Communicates regularly with the case team for the child/youth as well as other stakeholders for the child/youth
- Assists data personnel with appropriate paperwork related to placement
- Completes all documentation necessary to ensure that appropriate placements are located for clients. This includes CMS and all other documentation platforms utilized
- Maintains contact with all child placement agencies and foster parents as well as kinship staff to develop and maintain positive rapport
- Establish relationships and resource list for homes specializing in care for high needs children/youth
- Adapts to a flexible work schedule.
- Completes accurate and timely documentation as assigned
- Participates in Placement Coordinator holiday on-call rotation
- Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP's), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and all term of the contract between Saint Francis Ministries and DCF
- Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
- Ensures clients' rights are protected
- Is knowledgeable of and follows all safety procedures
