    Care Manager RN at Providence Portland Medical Center. This is a Full-Time (1.0 FTE), Day Shift position.


    RN Care Managers, in the acute care setting, provide a variety of services to appropriately help progress patients through the continuum of care.

    Key services include screening for post-acute needs; transition planning; care coordination; education; community care referrals; care planning in accordance with regulatory requirements; collaboration with the continuum care team; ongoing monitoring of plan effectiveness and reassessment of the need for Care Management (CM) services.

    Required Qualifications:
    Nursing degree/diploma required upon hire.

    Bachelor's degree in Nursing or higher within 3 years of hire.

    Upon hire:
    Oregon Registered Nurse License

    2 years acute care hospital or community based experience.

    1 year experience with electronic medical records or documentation systems.

    1 year home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience


    Preferred Qualifications:
    Bachelor's degree in nursing upon hire.

    The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors.
