Job summary
The Dietitian is primarily responsible for providing service in accordance with a plan of treatment and care plan for the Home Health Agency.This job offers the opportunity to work as a licensed dietitian in the home health setting, providing services to clients in the Twin Cities Metro area. The position requires observing and evaluating treatment effect and client's overt physical/emotional response to treatment goals, updating treatment plans based on evaluation and new orders received.
Key responsibilities include assessing rehabilitative status of clients, developing appropriate treatment plans, communicating with interdisciplinary teams, documenting client treatment and response accurately using established guidelines. This role also involves honoring client confidentiality rights privacy reporting maltreatment expectations following emergency response procedures identifying home safety issues notifying RN case manager or clinical manager concerns or issues demonstrating proper handling lifting transfers injury protocols overview process types reports forms follow-up required escalating event other duties assigned.
