- Collaborate with healthcare professionals to ensure optimal medication management for longterm care residents.
- Review medication orders and provide accurate and timely dispensing of medications.
- Conduct medication regimen reviews and offer recommendations for therapy optimization.
- Provide clinical support and guidance to nursing staff and other healthcare team members.
- Ensure compliance with all state and federal regulations governing pharmacy practice in longterm care facilities.
- Participate in quality improvement initiatives and medication safety programs.
- Maintain accurate medication records and documentation.
- Monitor medication usage trends and assist in the development of pharmacy policies and procedures.
- Provide patient education on medication use, side effects, and drug interactions.
- Perform other duties as assigned by the Pharmacy Manager or Director of Pharmacy.
- Active Indiana Pharmacist license in good standing.
- Prior experience in longterm care or infusion pharmacy is preferred but not required.
- Strong clinical knowledge and understanding of pharmaceutical care in the longterm care setting.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Commitment to providing compassionate care to longterm care residents.
- Flexibility to work Monday to Friday dayshifts (8hour shifts).
- Fulltime availability for an immediate start.
- Holds an active IN pharmacist license.
- Available for fulltime work immediately.
