    North Suffolk is looking for a Clinical Coordinator to join our Hanton House team The Clinical Coordinator works with the Program Director to lead the program in the assessment, treatment planning and service delivery processes and is responsible for determining persons' need for rehabilitative services.

    They oversee the development of the persons' treatment plan and subsequent reviews and revisions to ensure the goals, objectives and interventions reflect the Clinical Formulation-Interpretive Summary and the prioritized assessed needs.

    Services are provided based on Recovery tenets and are aimed to promote overall health and well-being of persons served. The Clinical Coordinator provides leadership and assists clinical supervision and guidance to direct care staff.


    What you'll do:


    Engage people and partner with them to improve their health and wellness, achieve symptom stabilization and self-management, develop and/or restore skills impacted by mental illness/substance use and live a self-directed life.

    Assist people with psychiatric and substance use disorders to identify personal priorities, preferences, strengths, and interests in order to help them establish goals that support a life in recovery.

    Instill hope by engaging in positive interactions that promote recovery and empowerment.

    Provide clinical services in timely and flexible ways that are consistent with the principles and practices of trauma informed care and person centered approaches.

    Collaborate with persons to help them to write goals with specific action steps in order to develop effective

    treatment/rehabilitation

    recovery plans.

    Participate in the development and implementation of Safety Plans and Crisis Prevention and Response Plans in order to help prevent and mitigate risk factors.

    Be knowledgeable of and use evidence based practices including motivational interviewing, SBIRT, Harm Reduction, Stages of Change and Housing First and other relevant best practices.

    Provide supervision, training, mentoring and modeling of evidence based practices with direct care staff.
    collaboration with other members of the COE team, participate in comprehensive assessment of psychiatric history.
    Document each person's progress in the confidential record according to established methods and procedures.
    Carry out the assigned interventions and action steps of the treatment plans.
    Participate in providing on-call services including on evenings and weekends per on-call schedule.

    Organize and lead individual and group social and recreational activities to structure each person's time, increase social experiences, and provide opportunities to practice social skills and receive feedback and support.

    Prepare information and testimony related to guardianship hearings, including Rogers type and case summaries in preparation for clinical and risk reviews.

    Serve as a liaison to courts, including filing for Section 35s.

    What you'll need:

    Education:
    Masters degree in Social Work or Mental Health Counseling required


    Experience :


    Previous experience in the provision of clinical or rehabilitative services to adults with chronic and persistent mental health issues and/or substance use disorders preferred.


    Skills:
    Must possess a valid Mass driver's license and reliable transportation.
    Must be MAP certified and CPR/1st aide certified within the first three months of hire.
