• RN Case Manager with Home Health experience
• Manage case assignments, develop service plans, track progress, and execute case closures.

* dvocate for clients with advice and information about treatment options and clinical status, evaluating medical necessity and appropriating resources.

• OASIS documentation.
• Track and record case information, complete forms, and produce reports.
• Facilitate care and treatment via scheduling, information sharing, and case coordination.
• Assess and act accordingly in response to treatment barriers and psychosocial issues.

*Job Requirements:

• RN License required
• BLS (AHA) required
• Must have experience in home health and with OASIS documentation.

Pay:
$ $72.00 per hour

Expected hours: 40 per week


Schedule:

• Day shift
• Monday to Friday

Experience:

• Nursing: 1 year (Preferred)
• Case management: 1 year (Required)
