- Key Responsibilities:
- Coordinate all systems/services required for patient-centered care team approach to care coordination;
- Conduct initial and ongoing assessments, initiate disease management protocols, and determine and manage patient outcomes;
- Ensure continuity of care through discharge planning, utilization of resources and analysis of variances;
- Facilitate and support quality care in relevant clinical area of expertise;
- Participate in continuous quality improvement activities in collaboration with other health care team members.
- Bachelors degree in Nursing from an accredited institution preferred;
- At least one-year experience in patient care coordination;
- Active RN license in the state where employed;
- CPR/BLS certification required

Description
RN Coordinator for Patient Care
Our hospital seeks an RN Coordinator for Patient Care who will be responsible for coordinating patient care services. The Coordinator will work with physicians, nurses and other staff to ensure that quality patient outcomes are achieved. They will conduct assessments, manage treatment plans, and ensure effective communication among health care team members, patients and their families. The successful candidate must hold a RN license and have at least one-year experience in the field. A Bachelors degree in Nursing from an accredited institution is preferred. The job requires light work with occasional physical risks.
