    Description

    Care Coordinator

    We are seeking a dynamic and results-driven Care Coordinator to serve as the face of Access TeleCare in the market.

    • Collaborate with clinical leadership to develop and document client-specific clinical workflows.
    • Partner with providers and clients to ensure key quality metrics are being met.

    The ideal candidate will have excellent organizational and interpersonal skills, experience working in health care services or case management, and proficiency in EMR systems such as Netsmart Credible Epic etc.
