    The Care Navigatoris the first face members and patients see when entering a clinic and is instrumental in helping members feel welcome. The Care Navigator will provide a variety of clerical, clinical and membership enrollment tasks. Daily responsibilities will include helping coordinate patients health needs, social services and answering non-clinical questions. It will also include partnering with the Patient Advocate(s) on events and seminars for Residents and Members, engaging directly with individuals, and working to help the rest of the team coordinate and provide additional services. This role combines traditional and effective clinical operations management, problem-solving, and organizational skills in a truly rewarding, multi-channel growth role.

    Essential Functions:

    The Care Navigator will apply clinical and administrative training to assist the Primary Care Provider in implementing the care of patients. These tasks include:

    • Checking our patients in and out of their visits with the Provider
    • Registering patients, including gathering copies of insurance cards, driver's licenses or identification cards, authorizations, referrals and other medical records managing the upload and routing documents in Athena, our EMR
    • Scheduling patient appointments and updating patient information within Athena
    • Working with patients and other physician offices to obtain medical records
    • Managing the phone line for the clinic e.g., appointment reminder calls, refills, following up on referrals, and tracking of lab orders.
    • Completing patient intake, including vitals and history for clinical encounters
    • Venipuncture and completion of basic lab tests, e.g., UA, rapid strep/flu at the direction of the Provider
    • Removing sutures and changing dressings at the direction of the Provider
    • Administering medicine and injections at the direction of the Provider
    • Preparing and assisting with medical procedures and exams
    • Perform Electrocardiograms (ECG) for providers to read
    • Providing basic patient education, e.g., diet, exercise, and providing iFOBT kits
    • Assisting with Population Health Management, e.g., patient outreach and panel list management
    • Going above and beyond in all circumstances to ensure members receive the best care
    • External vendor lab management, including but not limited to Quanum for ordering supplies and retrieval of lab results

    Education & Experience Requirements:

    A Certified or Registered Medical Assistant or a State Licensed Practical Nurse

    3 years of clinical and administrative experience

    3 years of experience working in an Electronic Medical Record System

    Basic Life Support (BLS) certification

    Phlebotomy experience and certification required or willing to obtain in six months

    Strong computer skills and a basic knowledge of Microsoft Office

    Ability to work without direct supervision and perform tasks autonomously

    Ability to collaborate with professional colleagues as necessary to provide quality care

    Ability to consistently present a friendly, welcoming, and professional public-facing appearance

    Primary Care, Family Practice, or Geriatric experience preferred

    Experience working with Athena (EMR) preferred
