- Conduct comprehensive assessments, medical history reviews, diagnosis and treatment, health education, referrals, case management, follow-up, and documentation per Matrix guidelines
- Collaborate with healthcare professionals, family members, and caregivers
- Work in various settings including home visits, skilled facilities, and telehealth
- Master's Degree or equivalent experience with NP licensure
- Current RN and NP or PA licensure with prescription authority
- Board certification in a Matrix-approved specialty
- Minimum 1 year experience as a Nurse Practitioner/Physician Assistant
- Strong computer skills and EMR software familiarity
- Excellent communication skills
- Positive, outgoing personality and ability to adapt to change
- Valid driver's license, access to a car, and automobile insurance
- Travel to meet patients in their location

Nurse Practitioner/Physician Assistant
PRN/Part Time – Home Risk Assessments
$1,500 Sign On Bonus

About the Position:
During visits that can last up to one hour, Matrix providers assess a member's current health, medical history, medication adherence, social environment, and other risks. This in-depth assessment provides valuable insights into a member's overall health and well-being that are often missed during routine office visits. The Matrix Comprehensive Health Assessment enhances the quality of care and helps address multiple care gaps in a single visit.

Nurse Practitioner or Physician Assistant - PRN/Part Time - Home Risk Assessments
Type: PRN/Part Time - Flexible-Nurse Practitioner
PAY: $90.00 or $115.00 per completed Assessment
Location: Home, SNF, Telehealth, and other environments in Binghamton, New York
Hours: Flexible scheduling including days, evenings, nights, and weekends
Benefits: PRN/Part Time providers receive 401(k) and limited voluntary benefits, along with a mileage reimbursement plan
