- Engage with members regularly to offer ongoing support
- Coordinate community resources with a focus on medical, behavioral, violence prevention, and social services
- Conduct risk assessments and crisis interventions, ensuring member safety
- Explore care options and develop service plans utilizing benefits and community resources
- Educate members on various topics including mental health, abuse prevention, and available resources
- Facilitate appointments, follow-up services, and provider visits
- Act as a liaison between members, families, physicians, and facilities/agencies
- Report critical incidents and care quality concerns
- Maintain accurate and confidential records of all case management activities
- Communicate effectively with members, staff, and the treatment team
- Ensure high-quality documentation for optimal member treatment
- Bachelor's degree in psychology, social work, or related field required
- Master's degree preferred
- At least two (2) years of relevant experience
- Familiarity with local resources, mental health, substance abuse disorders, and social issues
- Experience working with at-risk youth is a plus
- Completion of Crisis Intervention Training

Job Description
We are looking for Care Managers based in Tennessee to join our Teen Suicide Prevention Program team. These roles involve some local travel, up to 50%, so you must reside in Tennessee with reliable transportation.
At Mindoula, our Care Managers, supported by advanced technology such as a mobile engagement app, specialized psychometrics, and predictive analytics, play a crucial role in our success. As we expand rapidly into various business sectors across multiple states, we are in need of experienced Care Managers for our Suicide Prevention Program.
Mindoula offers virtual and in-person assistance to individuals and families dealing with suicidal thoughts or attempts. Our Care Managers strive to enhance their lives and well-being by connecting them with appropriate healthcare and community services.
Details: This is a full-time position with working hours from 11:00am to 8:00pm Eastern Time.
