    Description

    The Bungalows at Jonesboro is looking for Wellness Nurses to be part of their team!


    The LPN (Nurse MedTech) will directly report to the Wellness Director.



    Shift Details:

    • 12-hour shifts: Days, Evenings, Overnight

    Full-Time and PRN positions available


    LPN - Licensed Practical Nurse


    PURPOSE

    The Wellness Nurse/Nurse Med Tech is responsible for overseeing the health and well-being of residents, providing leadership support, and assisting with resident care and services.

    Responsibilities involve medication administration, communication with support services for residents and families, and upholding clinical quality assurance while promoting wellness - core components of Phoenix programs and services.


    PRINCIPLE DUTIES AND RESPONSIBILITIES


    LPN - Licensed Practical Nurse

    • Resident Care
    • Identifying resident needs through assessment/Personalized Service Plan (PSP) process
    • Ensuring proper documentation for residents with changes in clinical care needs
    • Assisting the Wellness Director in completing clinical sections of monthly wellness visits and PSP
    • Updating Wellness Director/Executive Director of changes in medication or service levels
    • Obtaining weights and vital signs monthly for each resident
    • Providing direct resident care when necessary

    EDUCATION/EXPERIENCE/LICENSURE-CERTIFICATION


    LPN - Licensed Practical Nurse

    • Must be a Licensed Practical Nurse or Registered Nurse
    • Minimum one year of experience working with elders or disabled individuals
    • Demonstrated Leadership capabilities
    • Knowledge and experience in Assisted Living industry, Dementia care, or Skilled Nursing
    • Ability to work weekends, evenings, and flexible hours
