Heart Failure RN Navigator at Providence St. Vincent Medical Center. This is a Full-Time (1.0 FTE), Day Shift position.
The Heart failure navigator will serve as a consistent resource to support optimal care delivery for heart failure patients with emphasis on achieving guideline based on best practice care, focusing on coordination and seamless transitions of care as patients move to the next level of care.

Oregon Registered Nurse License
Upon hire:
National Provider BLS - American Heart Association
3 years of Cardiovascular experience or similar healthcare experience.
Preferred Qualifications:
Bachelor's Degree, Or
Master's Degree
Experience with cardiovascular patient care in the inpatient or ambulatory setting.
