- Review patient profiles to assure safe and effective medication therapy
- Responsible for dispensing of medications, monitoring and reporting drug safety based on current clinical and institutional guidelines, as well as patient-specific criteria
- Oversees technician practice/activities as per regulatory requirements.
- Responsible for mediation storage and documentation
- Actively participates in clinical services (e.g., stewardships, cost-effective, patient experience)
- Adheres to safety requirements in the handling and preparation of sterile and non-sterile medications and other hazardous agents according to current guidelines (USP 797, USP 800, ASHP, NIOSH)
- Communicates effectively and appropriately with healthcare providers and caregivers and assures continuity of pharmaceutical care between shifts and among team members
- Participates in the training, education and mentoring of other pharmacists and technicians
- Participates in quality, safety and other project work related to medication management
- Graduated from accredited school of pharmacy
