Our Avance Care footprint spans across the Triangle (Raleigh-Durham-Chapel Hill) in North Carolina and recently has expanded to the Charlotte, North Carolina market. Avance Care is committed to delivering exceptional care with a strong emphasis on patient involvement in preventive care and treatment plans. Requirements: Nurse Practitioner or Physician Assistant with an active North Carolina State License Location: Rea Farms Charlotte, NC Explore and experience how Avance Care is providing professional and personal life balance:
- Maximum of 40 hours per week (including breaks and administrative time)
- Expected average patient load of 2 visits per hour
- Collaborative environment with onsite lab, behavioral health therapists, and nutritionists
- Innovative, integrated use of telehealth
- Centralized care coordination, care management, and population health management
