    We are currently looking to hire a Per Diem Physical Therapist to join our team immediately. The Physical Therapist (PT) plays a crucial role in enhancing the lives of our residents by delivering top-notch aging services with excellence and innovation.

    • Promote and maintain residents' functional performance, physical mobility, and overall well-being through the provision of physical therapy services.
    • Develop and implement services aimed at restoring function, enhancing mobility, alleviating pain, and preventing permanent physical disabilities in residents affected by injuries, aging, or disease.
    • Direct resident participation in specific tasks to improve performance and overall health.
    • Adapt examination techniques and treatment plans according to resident's needs. Manage residents' cases effectively.
    • Ensure compliance with regulations, standards, and proper documentation procedures.
    • Take on an appropriate caseload and handle billing and insurance procedures correctly.
    • Look for opportunities to enhance services within the Rehab department.

    Essential Responsibilities:

    • Develop, establish, and evaluate individualized physical therapy treatment plans for WEL Residents across multiple sites.
    • Conduct evaluations and treatments focusing on activities of daily living and range of motion exercises to enhance residents' abilities.
    • Provide discharge planning to promote independence and maximize function.
    • Work collaboratively with the rehabilitation team to create activities tailored to each resident's needs.
    • Ensure compliance with federal, state regulations, and accreditation standards related to long-term care and rehabilitation services.
    • Document treatments, evaluations, and progress notes accurately and timely.
    • Attend required meetings and maintain positive relationships within the team and other departments.
    • Supervise and direct Certified Physical Therapy Assistants and Aides.
    • Follow safety policies and maintain confidentiality as per HIPAA regulations.
    • Communicate essential information regarding resident care and progress to the team.
    • Perform other duties as needed.

    Qualifications

    • Three to five years of experience in a skilled nursing facility setting.
    • Demonstrated ability to work effectively within an interdisciplinary team.
    • Bachelor's or Master's degree in Physical Therapy from an accredited school.
    • Current license as required in the state of Pennsylvania and registered with the American Physical Therapy Certification Board.
