Description
JOB TITLE:
Financial Case Manager
STATUS:
Hourly 37.5 hours weekly, full-time
$19.00 to $20.50 per hour
SUMMARY
Come join us as we take on the challenges and opportunities of longer life and assist older adults in managing their financial affairs and maintenance of personal independence within a fee-based service.
This position is responsible for maintaining an average caseload between 30-35 clients and is responsible for general management of financial affairs including bill paying and helping plan and maintain long-term financial security of clients.
The Case Manager is responsible for meeting with clients a minimum of two times annually and more frequently depending on the level of need.
This is a community -based service and the Case Manager will meet with clients in a variety of settings including the individual's residence, skilled nursing facility, hospital or financial institution.
The case manager delivers services in a proactive rather than reactive framework.DUTIES AND RESPONSIBILITIES
Assess and investigate financial situation of clients.
Determine and plan appropriate actions that will assist client in becoming as financially secure as possible.
Provide full bill paying and budgeting services to ensure long term security.
Assist with financial decision- making.
Provide referrals to other community resources assisting the client to remain as independent as possible.
Maintain and manage all records, case notes, billing, reports and metrics as required ensuring accuracy and compliance with agency departmental policies and procedures, including state and federal tax laws.
Appropriately bill clients for services rendered.
Participate in all Lifespan staff meetings, trainings, continuous process improvement and in the implementation of agency mission and vison
Other duties as assigned by supervisor.
Requirements
QUALIFICATIONS
EDUCATION:
Bachelor's degree required.
EXPERIENCE:
2 years' banking or financial services experience required. Must have strong financial acumen and a working knowledge of community resources. Expertise in MEDICAID and other benefit progress preferred. Prior experience working with and advocating for older adults preferred.
