    POSITION FEATURES:

    • Experienced Home Therapy Registered Nurse
    • Preference for experience with multiple dialysis modalities (HHD & PD)


    PURPOSE AND SCOPE:

    Integral part of the Home Therapies dialysis healthcare team, ensuring high-quality care for home patients in line with company standards. Dedication to the Quality Enhancement Program (QEP) and Continuous Quality Improvement (CQI) initiatives, aiming to enhance patient satisfaction and outcomes. Actively engages in process enhancement to support patients in achieving Quality Enhancement Goals (QEP).

    PRINCIPAL DUTIES AND RESPONSIBILITIES:

    Staff Related:

    • Conducts safe training and supervision of home dialysis therapies, in adherence to established standards and regulations.
    • Participates in staff rotation to ensure sufficient coverage.
    • Engages in staff training and orientation programs.
    • Attends all mandatory staff meetings.

    Quality:

    • Delivers outstanding patient care quality as per company standards.
    • Provides Continuous Quality Improvement (CQI) data for review.
    • Actively participates in CQI meetings.

    Education:

    • Assists in identifying and assessing potential Home Therapies Program candidates.
    • Provides comprehensive training to patients and families on therapy equipment operation.
    • Educates on ESRD and Kidney Options Program.
    • Evaluates patient readiness and suitability for home dialysis.
    • Continuously educates patients on dialysis and related conditions.

    Dialysis Treatment:

    • Ensures safe and effective delivery of home therapy to ESRD patients.
    • Monitors patient response to dialysis treatment.
    • Communicates patient issues to physicians promptly.
    • Coordinates patient transfers when necessary.

    Laboratory Related:

    • Accurately manages physician lab orders and results.
    • Adheres to lab specimen collection and reporting protocols.

    General Duties:

    • Maintains emergency procedure proficiency.
    • Administers prescribed medications and maintains accurate records.

    Maintenance/Technical:

    • Maintains expertise in equipment operation and troubleshooting.
    • Guides patients in equipment maintenance.
    • Coordinates equipment delivery and maintenance.

    Medical Records & Documentation:

    • Provides comprehensive patient documentation.
    • Ensures compliance with admission, transfer, and discharge policies.
    • Completes required reporting.

    Other:

    • Handles assigned duties effectively.

    PHYSICAL DEMANDS AND WORKING CONDITIONS:

    • Combination of desk work, patient interactions, and travel.
    • Work environment typical of a healthcare facility.
    • Possible exposure to infectious materials.

    SUPERVISION:

    • No direct reports

    EDUCATION AND REQUIRED CREDENTIALS:

    • Graduate of an accredited School of Nursing (R.N.)
    • Current state licensure

    EXPERIENCE AND SKILLS:

    • Minimum one year medical-surgical or ICU nursing experience.
    • Minimum one year dialysis experience.
    • Completion of dialysis training course.
    • Proficient in CPR and Icd-9 coding.
    • Effective teaching skills.
