    The Nurse Navigator is responsible for assisting Oncology patients by navigating them through the health care system by helping to provide holistic care. During this process, the Navigator will collaborate with other members of the healthcare team, by promoting lifelong learning to improve the care of patients with a past or current diagnosis of cancer. The Navigator will address and connect the patient and families to any financial, psycho-social, educational, spiritual, and referral needs. In order to accomplish this, the navigator will oversee and direct patient centered programs to maintain the patients' health and well-being; there will be daily communication with the Oncology/Infusion Director.
    • Performing triage in the clinic/infusion setting and being available for other nursing staff and physicians in this setting, as needed (covering nursing shifts, relieve staff for lunches, assist with peer to peers, denials, training new staff, set patients up on clinical trials, as the need arises).
    • Orchestrate the morning huddles; collaborates with case managers/navigators across the healthcare system.
    • Participates in tracking of metrics and patient outcomes, collaborating with involved care providers; documenting, evaluating, and reporting findings
    • Promotes a patient and family centered care environment for ethical decision making and advocacy for patients with cancer; assisting with Advanced Directives, keeping current on legislation, Medicare and Medicaid guidelines, as needed.
    • Helping to screen for high-risk patients that would benefit from genetic testing.
    • Obtains or develops oncology-related education materials for patients and staff, as appropriate.
    • Playing a role in community education.
    • Assisting Director with Tumor Board preparation, staff meetings, and in the interview process of new employees, as well as attending meetings to represent the Oncology/Infusion Clinic, as needed.
    • Performing all chemotherapy teachings and Immunotherapy.
    • Establishes and maintains professional role boundaries with patients, caregivers, and the multidisciplinary care team in collaboration with Director as defined by job description.
    • Identifies potential and realized barriers to care and facilitates referrals as appropriate to diminish barriers; Hospice, Transitions, Palliative care, medication adherence, writing of opioid contracts.
    • Assisting with updating policies and procedures as needed.
    • Assist with marketing needs for the Oncology Department, if needed.
    • Navigators will be in the charge position in the absence of Director.
    Requirements

    4 days/week, 9 hours/day
    • Must be currently licensed to practice as a RN in the State of Colorado.
    • Two years of recent hospital experience required.
    • Previous experience as a Case Manager or Oncology experience-required.
    • Basic Chemotherapy/Biotherapy course completed and OCN preferred.
    • Proficient in triage.
    • Able to assist Clinic Nurses or Infusion Nurses when needed.
    • Computer Skills- Technical, Computer and EMR experience-preferred.
    • BLS required upon hire.
    • ACLS required within 6 months of hire date.
