    Soleo Health is looking for a Per Diem Licensed Practical Nurse (LPN) to assist infusion patients near Athens, GA. Join us in Simplifying Complex Care!

    The Position:

    The Licensed Practical Nurse (LPN) travels for labs and dressing changes in Athens. The LPN offers primary patient care, assessments, care planning, evaluation, and education for patients receiving infusion therapy. Responsibilities include:


    • Provide direct care for assigned patients
    • Evaluate and educate patients for safe therapy administration
    • Evaluate the home environment for care safety
    • Develop patient Care Map and evaluate therapy response
    • Coordinate care team communication
    • Document all communications

    Schedule:

    • Minimum 16 hours per week
    • Create your own schedule

    Requirements:

    • LPN license
    • Minimum 3 years nursing experience with infusion background
    • Prefer ICU and/or Cardiac Care experience
    • Excellent customer service skills
    • Proficiency in Microsoft Excel, Word, and Outlook
