Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care.

~ Provides skilled nursing care to patients in accordance with the p lan of c are and agency policies.

~ Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.

~ P romotes and maintains patient health through teaching and appropriate rehabilitative measures.

~ Assists patients in learning appropriate self-care techniques.
