- Meet program participant outcomes by determining the objectives needed to reach the treatment goals and identifying service needs that may or may not include contact with individuals, families, guardians, and/or significant others
- Manages a caseload and oversees the coordination of all Case Management (CM) services to include the development of a (MIRP), completion of the behavior health assessment; acuity determination and crisis plan development and reviews.
- Maintains referral network with other human service agencies in the community and makes the appropriate referrals to other programs, community organizations, etc.
- Make collaborative outreach attempts, including transportation of program participants as needed, to coordinate member's care as necessary.
- Participate in crisis intervention services with individuals including mental status and risk assessment, and linkage to crisis teams and hospitals. Assesses needs, and develops and coordinates written crisis plans, when indicated, in coordination with client's psychiatrist or APRN-RX
- Coordinates and provides case management with program participants who are eligible for the ACT program
- Submits Monthly reports
- Attend Weekly Homeless Intensive Case Management and ACT Team Meetings
- Engage with program participants 1-2 times per week.
- Links clients with access to a psychiatrist or APRN-RX for direct care and consultative services
- Facilitates the provision of a minimum of one hour per week of skill building, psycho-education, supportive counseling or social skills, and ensure that the client has access to transportation services as needed, which may involve escorting/accompanying clients in the community
- Ensures program participants understand their rights and limitations as recipients of mental health services, including informed consent, and the risks and benefits of treatment, on an ongoing basis in with accordance and directives from assigned guardians
- Attends all scheduled supervision, departmental staff and General Staff Meetings
- Respects and maintains the confidentiality of the guests/clients served.
- Perform other duties and accept responsibilities as assigned.
- Frequent exposure to stressful situations due to the population of focus, including but not limited to: individuals with substance abuse issues and disorders, and severe and persistent mental health issues. Delivery of services are at HOPE Services Hawaii facilities and in a variety of locations including the client's home, other provider offices/locations, and street based settings, depending on the needs of the individual.

- Ability to handle administrative responsibilities of position including effective documentation skills.
- Ability to effectively interact with professionals, partnering agencies and clients with severe and persistent mental illness and/or substance abuse problems.
- Able to honor individual preferences at various stages of recovery.
- Effective problem solving and organizational skills and computer proficient in Microsoft Word, Excel and Access and/or database applications.
- Bachelor's degree from an accredited university or college, preferably in social work, psychology, human service or related field with minimum 12 semester credit hours in courses such as psychology, counseling, criminal justice, human services, social work, social welfare, sociology or other behavioral sciences.
- 2 years' experience navigating healthcare, human services, and substance abuse care systems.
- CPI certified or able to certify through the organization.
- CPR/First Aid/AED certified or able to certify through the organization.
- Current Hawaii Driver's license, preferred clean traffic abstract, and access to an insured vehicle for your use to conduct business.

Overall Responsibility:
Provides intensive case management services for people experiencing homelessness and severe mental illness (SMI), who would benefit from psychiatric treatment, but are unable to assist in their own treatment and recovery. Provides quality case management to include supportive counseling, outreach, psychiatric stability housing, life skills and education, family and community reunification support, service coordination/advocacy, and crisis support. Works closely with the Assisted Community Treatment (ACT) Coordinator and stakeholders regarding guardianship and petitions, when appropriate. Identifies and secures access to intense
community-based services according to program expected outcomes, and resources available. Assists program participants to obtain the support, skills, and services necessary to improve their quality of life, and their ability to function within their communities. Outcomes related to this role include successful ACT petitions, reducing hospitalization, arrest and/or recidivism to homelessness.

Scheduling requirements:
General business hours are 8:00am - 4:30pm Monday through Friday, but this 40-hour a week, exempt position offers flexible scheduling. There will be scheduled meetings (online and in person), and there will be times you are required to work evenings and weekends.
