We are currently seeking an Occupational Therapist - OT to join our Home Care team

Constellation Health Services is a 5 Star CMS Rated Home Care Agency in NY

This is a field-based position where the OT will provide home-based therapy.


Responsibilities:
Design and implement custom therapy programs to restore patients' independence at home.
Evaluate the patient's functional status and home environment for hazards or barriers.
Participate in the development of the individualized plan of care and identify any equipment needs.
Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
Maintain and process all documentation in compliance with agency standards utilizing the electronic medical record-keeping system.


Qualifications:
Licensed Occupational Therapist - OT in the state of New York with at least 1 year of clinical experience;
Home Care experience is preferred.
Oasis experience is strongly desired.
Have reliable transportation and be willing to drive to cover an assigned territory.

Some weekend days are required, and summer and winter holiday commitments as needed
