    Residential Hospice is seeking a Hospice Weekend Clinical Supervisor who will directly supervise clinical staff and work collaboratively with the Hospice Manager to ensure the integrated functioning of the Hospice program for optimal benefit of the patients, families, and facilities we serve.

    Additionally, this person will work to improve quality of care and service, work performance and department processes.

    The Hospice Weekend Clinical Supervisor will perform nursing duties in the provision of skilled nursing assessment, planning, and coordination of services in order to maximize the comfort and health of patients and families.


    Hospice Weekend Clinical Supervisor Responsibilities:

    • Supervisor Weekend After Hours from Friday 5pm through Monday 8am
    • Directly supervise the After Hours and Weekend nursing teams
    • Manage the weekend schedules for all clinicians
    • Monitor and manage reports, volume and visits
    • Manage patient complaints
    • Approve medications and DME
    • Active involvement with Care XM/Total Triage
    • Ensure productivity goals of clinical staff are met
    • Ensure case management guidelines are being followed by each team member
    • Assign patients to multidisciplinary team and adjust schedules as needed
    • Serve as the first line for resolution of concerns and issues for the staff
    • Train, develop, and mentor staff, including creating and recruitment, administering orientation process and patient assignment, providing employees with continuous training and education, and performing employee reviews, exits, counseling, and meetings
    • Communicate regularly with support staff to assess current needs, determine level of accountability, assign patients, and provide supportive discussion related to areas of responsibility and job duties
    • Participate in on-site reviews and inspections
    • Develop staff to meet quality, safety, and competency requirements by assisting with training and professional development of new and current staff, annual joint visits, and setting up vacation coverage
    • Work in tandem with leadership to ensure consistency among staff with any changes or updates in ALFs, SNFs, or MD office requirements
    • Assist with the orientation process including setting up supplies, precepting new hires, and setting up the orientation schedule with other disciplines (MSW, Chaplain, Marketing, Volunteer, etc.)
    • Initiate communication with attending physicians, other Hospice team members, and agencies as needed to coordinate optimal care and use of resources for the patient/family
    • Support Nurse Case Manager in obtaining data on physical, psychological, social, and spiritual factors that may influence patient/family health status and communicate in team meetings to update the Interdisciplinary Plan of Care
    • Maintain up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated and process orders and changes are reflected as they occur
    • Act as administrator-on-call if assigned

    Hospice Clinical Supervisor After Hours Requirements:

    • Licensed RN or BSN with current state license
    • Minimum of 2 years of Hospice experience
    • Minimum 1 year of supervisor experience
    • Familiarity with all regulations pertaining to Hospice operation to ensure compliance
    • Ability to maintain a high level of competency, growth, and development
    • Current RN License and CPR in good standing
    • Ability to work independently and within a multidisciplinary team
