    Patient Care and Safety in Anticoagulation ServicesJob Summary:

    Sanford Health is seeking an experienced pharmacist to join our team providing pharmaceutical care services for the outpatient Anticoagulation Clinic (AC). The successful candidate will be responsible for staffing the anticoagulation clinic, working closely with a team of five nurses and an ambulatory care pharmacist. As part of a rapidly expanding ambulatory care program, this role offers opportunities to provide additional pharmacy services.

    In this critical position, you will assess medication use systems, identify areas for improvement in anticoagulation use safety, and implement practice changes as needed. Your primary responsibility will be managing AC patients by conducting follow-up monitoring and necessary interventions to ensure enhanced safety and effective outcomes associated with outpatient AC.

    Patient Interaction Skills

    We are looking for an individual who can demonstrate excellent communication skills. You should have the ability to understand age-related patient information relating to pharmaceutical needs across various patient demographics including neonatal pediatric adolescent adult geriatric patients qualifications skillset resumes In addition we expect that candidates possess strong problem-solving skills leadership abilities teamwork collaboration time management organizational performance quality control inspection evaluation assessment developmental research
