    Summary

    Responsible for assisting patients with coverage options and applications including but not limited to Medicaid, Medicare, and Affordable Care Act Plans. Acts as liaison for patients who are referred into and out of FCC clinics to help ensure adherence to treatment plans. Assists patients with access into social workers, referral providers, labs, and other health care entities.

    Essential Duties and Patient Centered Responsibilities include the following. Other duties may be assigned.

    Communicate cross culturally to educate clients/community members on health issues, increase knowledge of preventative health care, and increase knowledge and utilization of available health care resources.

    Provide a link between clinic, individual client, and the community network of service providers to increase collaboration and use of resources.

    Verify eligibility for programs, previous enrollment history, demographics and current health status of each member.

    Provide assistance to clients to enroll for services, including completion of forms/applications required to receive needed services, including but not limited to Medicaid, Insurance Marketplace, CHIP, TANF, SNAP and My Chart.

    Provide information and assist with referrals to appropriate community agencies and resources and serve as patient advocate to help clients navigate the medical system and access needed follow-up care.

    Assist clients with arranging/ensuring follow-up medical appointments, including transportation when necessary.

    Provide education for patients and their families individually.

    Organize and conduct health education/outreach activities and screenings to increase patient enrollment.

    Know, understand, and adhere to FCC policy related to the patient's rights for confidential care.

    Maintain appropriate documentation within protocols and guidelines.

    Have ability to work flexible hours, including possible evenings and weekends.

    Participates in CPI program. Maintains compliance, is knowledgeable, and utilizes the guidelines of all state and federal laws and regulations as they pertain to the position including, but not limited to; Health Insurance Portability Accountability Act (HIPAA), Scope of Practice, Accreditation Standards, Occupational Safety and Health Administration (OSHA), Bureau of Primary Health Care/ Health Resources and Services Administration (BPHC/HRSA), National Committee for Quality Assurance/Patient Centered Medical Home (NCQA-PCMH), Centers for Medicare and Medicaid Services (CMS), Clinical Laboratory Improvement Amendment (CLIA), Joint Commission on Accreditation of Healthcare Organizations (JCAHO), and other HRSA related regulatory functions and/or agencies that Family Circle of Care participates in.

    Education and/or Experience

    High School Diploma or GED required.

    Two years of health/clinical experience or equivalent health-related education required.

    Computer Skills

    To perform this job successfully, an individual should have knowledge of Microsoft Word Processing software; Microsoft Excel Spreadsheet software and Electronic Medical Record System Database software experience, EPIC preferred.

    Language

    Verbal and written bilingual skills (English/Spanish) strongly preferred.

    Certificates, Licenses, Registrations and Other

    Community Health Worker Certification from the Texas Department of State Health Services preferred. If not certified, must obtain certification within 6 months of the date of hire. Must have reliable transportation, a valid TXDL, and ability to produce a copy of current auto insurance certificate. Current CPR Certification and all required renewals preferred.

    While performing the duties of this Job, the employee may be exposed to airborne particles, infectious disease, biohazardous waste, blood, and bodily fluids. Employee is required to follow all established blood born pathogen and hand washing procedures. The noise level in the work environment is moderate.

    This position functions within the community and will include travel within the immediate counties which Family Circle of Care serves and driving between cities.
