Saint Louis MO US Saint Patrick Center provides opportunities for self sufficiency and dignity to people who are homeless or at risk of becoming homeless Individuals and families build permanent positive change in their lives through safe and affordable housing sound mental and physical health employment financial stability Working collaboratively with individuals businesses government other service providers Saint Patrick Center will end chronic homelessness in the St Louis region through Housing First an evidence based practice that tells us to remove barriers do intake assessment early move people into housing offer them support to achieve their goals Position Summary Catholic Charities of the Archdiocese of Saint Louis through our service line of Saint Patrick Center is hiring a Behavioral Health Counselor for the Shamrock Program plays a meaningful role in helping individuals move from homelessness toward stability independence hope This position embraces a compassionate wrap around philosophy giving counselors the opportunity to provide empowering individual group therapy that truly meets clients where they are You will work collaboratively with clients to build personalized Treatment Plans of Care that honor their strengths goals unique recovery journeys In this role you will help clients reconnect with family natural supports community resources stabilize In addition you will coordinate tailored services including mental health care substance use support affordable housing assistance ensure each client receives holistic care deserves Engage closely with service providers across home community settings create cohesive supportive network every client Document progress celebrate successes play vital part transforming lives better Core working hours are Monday Friday am pm Overview And Responsibilities Conduct interviews clients develop treatment plan Provide ongoing counseling case management services caseload clients Contribute collaborate team ideas processes Assist housing placement desires client Assess strengths desires immediate long term needs client Develop contact family natural supports exist community Receive team input regarding employment housing mentoring substance use long term needs Assist daily running Shamrock Club treatment program Complete departmental paperwork Document referrals services provided client Case Worthy Service Point systems Daily updates electronic files Case Worthy Service Point include types client contact services internal external referrals made each Adhere strategies outlined ISS Treatment Plan Address client needs providing necessary referrals Coach solid well informed decisions Review clean entry exit data HMIS monthly run reports HMIS assigned Attend quarterly HMIS PATH User Weekly Housing Match Meetings Maintain update records as needed Participate weekly team meetings clinical supervision Carry small caseload MH SUD clients ICM Act point person all Shamrock housing referrals WHAMM Collaborate Program Assistant daily assist administrative tasks needed Keep Memorial Service list current Facilitate group education meetings as needed Network related agencies Respect confidentiality HIPAA regulations Ensure care model interacting all St Patrick Center programs Research additional benefits available enhance services Intake assessment new clients entering day treatment program Other duties assigned
