What You'll Do as a Home Health Physical Therapist:

Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.

Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.
Document all visits, assessments, and progress notes accurately and in a timely manner.
Support patients in maximizing their independence and improving quality of life—wherever they call home.
Master's degree in Physical Therapy from an accredited program.
Current PT license in the state of employment (or eligibility to obtain).
Valid driver's license, auto insurance, and reliable transportation.1+ year of experience in a hospital, home health, or outpatient rehab setting
~ In-home therapy delivery
~ Geriatric rehab, orthopedic or neurologic rehab
~ Post-acute care or chronic condition management
~ Working as part of a mobile or interdisciplinary care team
